Razer Nari headset disconnect from PC | Razer Insider

I brought Nari Headset this year in May, it worked perfectly until yesterday. It starts to disconnect from PC and flashing blue light. When I re-plug the dongle is works for a minute or two and disconnect again. I tried to submit ticket on Razer website because it still under warranty, but the submit button just not working at all.

I provided serial number that register in. Also has my video ready to explain the situation, but no matter how many times I click the submit button it just not working. 

please help with that it really bothers me because I have many other Razer devices and I worried so much if the warranty page is just a hoax.
