wired wolverine controller not working

Hi, my wired wolverine ultimate xbox controller isn't connecting to my xbox one or my pc. When i try to plug it in it flashes white twice then turns off. I have been using it for a year and i haven't had this problem before. I mad sure all the cables that connect the controller were plugged in properly. In addition i plugged the controller into my pc and the computer gives the message saying your usb isn't plugged in properly or is malfunction. I have had this issue for several days now please help.
