Razor Wolverine Tournament edition - USB error / App not working

Hi, When i plug in the controller to the pc i get the attached usb error. I have installed the wolverine app for xbox and when i launch this it comes up with blank controller.. One in 50 times i plug in the controller starts but and the app shows the controller menu, but when i move the controller it is unresponsive.... One in 100 it actually works and can play a game.... Tried all the usb ports, some issue... Very frustrating Serial is S/N @(Omitted for security reasons)

    Thanks, i've dropped you a response. Given my controller is under warranty can i return this please since its unusable at the moment. Regards
