Same here. Says the serial number and the product number don't match.

Figured out the problem. The Product Number printed on the mouse and displayed on the top bar code on the bottom of the box doesn't work with the form.



If you look at the P/N written in green section between the white bar code boxes, that will be the one you want to use. It will start with: P/N: RZ01



The main difference is what is printed on the mouse and top bar code is: P/No: RC30 and the last 4 digits are zeroes. The green section which starts with P/N obviously starts with RZ and on mine the last 4 digits are alphanumeric.
