My controller keeps disconnecting and will only work again if I unplug it and plug ut back in. Any way to fix ?

Did you already try using a different USB port or cable? I also suggest you test the controller on a different console or PC for further isolation. If you're experiencing the same issue, PM the serial number with a copy of your receipt so I can start a case for a replacement request.
