refund on fraudulent card

I have had this conversation before with someone else and I need to turn in all the paperwork required for my refund. I have everything I need but no way to turn it in. I have been given so many tickets I don’t even know if mine is open still. I need a ticket request for this information so I can get my 200 dollars back on a already used card.
