Playing Hitman:Absolution and killing everyone in the building, not just the target. Civilians and all. Same thing in Assassins Creed expect the stray dogs were caught in the cross fire as well.
EVE, I let a bunch of noobs come mining with us in null sec. Well, goons show up and we hightail it outta there, but the noobs weren't fast enough. Went back a few minutes later in a stealth ship, waited till the goons were gone, and salvaged all the wrecks.
I hate people like you.... So annoying. The most I've gotten banned from killed someone because of THAT reason was 2 hours.
I put do not enter signs on the path leading to the exit...no one could get out. I also picked up angry people and would put them in the lake. They usually got less angry.
other than the beginning of COD MW2.... not much lol its to hard.
what makes that gif even better is the Star that was just awarded floating in the back.... had to kill the baby penguin first though lol. " Here is your star for finding my baby..... " [kills baby] takes star. its like the precursor to GTA, pay a hooker, kill her, take back the money lol

For the longest time I was a softie like @kajira and for the most part I still am but man, as a healer... there are times... FEW times mind you (thankfully) in raid PuGs... when someone isn't following the plan, is told multiple times what to do and still doesn't do it... when I've chosen NOT to heal that noob. That's about as evil as i've been.
omg - me too!! It's exceptionally rare, but there have been times where I've been super frustrated and @sirjarren just said "well, then don't heal them ..." so I didn't!! And I hate to admit it, but it felt so good to watch them die because they were stupid!!
I did that all the time when I played WoW
My main was Druid, dual spec'd for tanking and healing, so if someone was being a dick, I'd either not pull mobs off of them, or not heal them depending on my role. Sitting on their face in bear form / curb stomping their corpse with Tauren racial was also very satisfying. And when WotLK came out, impaling their corpse with a faction banner was outstanding.
If they were REALLY being a dick, I'd have them kicked just before the final boss.
Also played an Undead Death Knight for a bit - NOTHING was more satisfying than using using Corpse Explosion, and then Cannibalize to the lifeless body of someone with a crappy attitude.
I still think the worst I've done was in a fallout game (original) after I'd beaten the game. I decided I was the rightful owner of everything, and proceeded to pickpocket everyone. Halfway through Shady Sands someone notices my pick pocketing and starts combat with me (I had been a very good/high karma character through the whole play through) I proceed to kill all the adults in the town using only thrown rocks. But, alas, one rock misses & hits (and 1-shots) a child, so that gives me that particular trait that games don't let you get anymore.
Then I proceded to wipe out all NPCs from the game (the hub took a while)
My karma didn't look to good after that.
Probably burn down houses with sims trapped inside in The Sims. Or leaving them to starve to death after removing all the doors and windows.
When playing Syndicate Wars, it is never possible that street walking civilians will never get killed. I think I have killed them with every single weapon in the game and there are well over 10. However, I think my most egregious killing spree was when I killed over 100 civilians after leveling 3 city blocks with the orbital bombardment weapon known as "Satellite Rain".
I did that too.

a friend and I teamed up in tf2 and made a kid rage quit the entirety of steam.
I... cheated on on Ashley in Mass Effect 2 because I forgot she existed. Oopsies.
While running the Grimrail Depot dungeon in the new WoW xpac, our groups healer was being less than a team player. He died during a boss fight but we managed it anyways, after the encounter, i revived him while standing right on the edge of a platform. as soon as he accepted he was blown off the edge and died again. it was very satisfying.
